This 1099 envelope is usually used for item number 9ONEPERF05.  The overall size of this envelope is 3-5/16 x 9-3/16.  The top window is 1" x  3 15/16" and is 9/16" from the left side of the envelope,  the bottom of the top window is 2 5/16" from the bottom edge of the envelope.  The bottom window is 1 1/8" x 3 15/16" and is  9/16" from the left side of the envelope, the bottom of the bottom window is 3/4" from the bottom of the envelope.  This envelope must be moistened to seal. 

This envelope can be used in high-speed inserting equipment.
