Certain Government Payments. 

Forms are 3-up on an 8 ½” x 11” sheet with a ½” side perforation and are printed on 20# laser paper. Order the quantity equal to the number of employees for which you need to file.
 
Use the 1099-G Copy A to print and mail payment information to the IRS.  Printed with required red scannable ink.  Mail as a batch to the IRS. 

Use the 1099-G Copy B to print and mail payment information to the recipient (payee) for submission with their federal tax return. 

Use the 1099-G Copy C to print and mail payment information to the state or for Filer’s files. Mail in a batch to the state.
