W2 G preprinted. All W2 G forms are 2-up on an 8 ½” x 11” sheet. and printed on 20# laser paper. The W2 G form is a six part form divided between the gaming establishment and the customer.  Three copies of the W2 G form are given to the gaming winner. One copy is for the winner's records (C), one copy is for the winner to send with the state income tax return (2) and the final copy is to be sent with the winner's federal income tax return (B). The remaining three copies are retained by the gaming establishment. One copy is for the gaming establishment's records (D), with the other copies sent to the state (1) and the IRS (A).
